What to Expect from a Penile Implant?

What is a Penile Implant?

Penile Implants are the most rewarding treatment options for men having Erectile Dysfunction (ED), This comes under the field of Urology. These are prosthetic devices that bendable or inflatable. These kinds of implants are prescribed, when all other forms of treatment have been tried and not provided any respite. Also, when it is clear that the reason behind Erectile Dysfunction is totally a medical one.

Who’s a Good Candidate for this Procedure?

An apt candidate for Penile Implant is someone:
·        Having a problem of persistent Erectile Dysfunction, which is impairing his sex life
·        Who has tried medications including Tadalafil (Cialis), Vardenafil (Levitra), Sildenafil (Viagra), and Avanafil (Stendra)
·        Has tried a penis pump – a vacuum constriction device
·        Has a condition, like Peyronie’s Disease, which doesn’t improve with other treatment.

What do you need to do to prepare?

The preparation includes a detailed evaluation about the patient’s body size, age, history of Kidney Transplant, any abdominal surgery, if the penis is circumcised, etc. Also, there are diagnostic tests and scans that are done.
·        Three-piece – multi component Inflatable Penile Implant
o   Easy to use, just have to pump it up when required and release the valve for deflating
o   The pump is bigger, and makes it easy to inflate
o   Totally concealed
o   A prosthesis giving a normal look
o   Very less post-operative pain
o   Maximum rigidity achieved through this
o   Less amount of damage caused to the penile flesh
·        Two-piece Inflatable Penile Implant
o   Easy to use but can be trouble for those having Arthritic issues or those having difficulty using their hands, because the penile pump is small and can be tricky to inflate it
o   Need to use the pump only for erection; otherwise down when not using
o   All the devices are within the body
o   Gives a little abnormal appearance
o   Flesh near the penile area can get damaged or become thin
·        Semi-rigid, Non-inflatable, semi-rigid or malleable rods
o   Very easy to use for men with restricted dexterity
o   Just need to bend it up when required and down when not using
o   Totally concealed
o   Very low priced
o   The penis is mostly semi-rigid and just needs to be adjusted as per need
o   There is post-operative pain as the penis needs to adjust itself to a persistent semi-rigid condition
o   Gives an abnormal look and feel
o   Leads to deterioration and thinning of flesh around the penile area
What happens during the procedure?

This procedure takes about 45 minutes to an hour and is done either under general anaesthesia or Spinal anaesthesia. The patient is given antibiotics through the IV line and the surgery site is shaved to lower the chances of infection.
·        A catheter is passed into the bladder via the penis for collection of urine during the surgery
·        A cut is made in the Lower Abdomen, below the head of the penis, or at the base of the penis
·        Then the surgeon stretches the spongy tissue present in penis, which fills up with blood during an erection. This tissue is located inside both the hollow chambers called the Corpora Cavernosa
·        The surgeon chooses the implant of appropriate size and then the implant cylinders are positioned inside the penis
·        For example, if the surgeon is implanting a two-piece inflatable device, then a pump and valve are placed within the scrotum. Similarly, for a three-piece device, the surgeon will implant a fluid reservoir under the abdominal wall through an internal incision
·        Once the device is placed aptly, the surgeon closes the incisions

What’s recovery like?

The patient is given pain relief medicines for the post-surgical pain and antibiotics to avoid infection. The surgeon will also give instructions about when the patient can continue with his usual regimen; sexual activity and other physical activities can be resumed after about 4-6 weeks. The stiches are removed two weeks after the surgery. The surgeon might recommend fully inflating and deflating the Penile Implants two times in a day. This helps the patient to get habituated in using the implants and stretch the area near the cylinders. During the healing phase, the surgeon might suggest keeping the penis pointed upwards on the lower abdomen to prevent downward curvature.

How effective is the surgery?

Erectile Dysfunction treatment and surgery is effective because:
·        Very high success rate and patient satisfaction
·        Rapid recovery
·        A very discrete treatment technique
·        Cost effective
·        Covered by maximum insurance plans
